When Burnout Is More Than a Bad Week

The World Health Organization’s ICD-11 entry on burn-out describes it as a syndrome conceptualized as resulting from chronic workplace stress that has not been successfully managed. The WHO is careful with the label: burn-out is included as an occupational phenomenon, and it states that it is not classified as a medical condition. ICD-11 names three dimensions: feelings of energy depletion or exhaustion, increased mental distance from your job or feelings of negativism or cynicism related to it, and reduced professional efficacy.

Two things follow from that. Burnout is not a diagnosis you will be handed, and it is not a personal failing either. What it produces is what therapy works on: emotional exhaustion, low mood, anxiety, broken sleep, and a slow erosion of confidence in work you were once good at.

It is also not simply stress with a longer name. CAMH’s description of career burnout calls it a state of emotional, physical and mental exhaustion caused by excessive and prolonged stress, and it draws a useful line between the two: generally, someone who is stressed tends to be over-reactive and hyperactive, whereas burnout presents more as disengagement and produces a sense of helplessness. Stress usually feels like too much coming at you. Burnout more often feels like nothing left to meet it with.

What Burnout Therapy Involves at 101 Psychotherapy

There is no standard programme here, because people arrive at very different points. Some need to stop the slide before anything else can happen. Others are still performing well and want to understand why the inside has emptied out. Burnout therapy is built around which of those is true for you, and it usually draws on three kinds of work.

Cognitive Behavioural Therapy for Burnout

CBT for burnout examines the thinking that keeps the cycle turning. The belief that you ought to be able to manage this. The sense that declining something will be held against you. The rule that rest has to be earned first. Changing how you think about the work is one of the things that shifts, and it is often what makes the practical changes stick. Mindfulness and Recovering Your Attention

Burnout narrows attention until most of the day is spent bracing. Mindfulness-based work is used to interrupt that: noticing the physical signals earlier, and rebuilding some capacity to be somewhere other than the next task. It tends to matter most for people whose sleep has gone and who describe themselves as unable to switch off.

Boundaries, Workload and Saying No

This is the practical half, and for a lot of people it is the harder half. Changing work patterns. Coping skills such as time management. Interpersonal effectiveness skills such as assertiveness and conflict resolution. Working out which meeting to decline is the easy bit. Declining it without spending the next three days justifying yourself is the skill. We go further into that on our assertiveness therapy page.

What Therapy Can Change, and What It Cannot

The CPA fact sheet says interventions should focus on both the individual and the environment, and that on the research evidence it appears individual-focused interventions are not sufficient to tackle severe burnout, because workplace stressors also need to be considered and addressed. Therapy for workplace burnout works on your half. It can help you recover capacity, sleep again, think more clearly about what you want, and hold a limit once you have set one.

What it cannot do is reduce your workload, replace a manager, or make an unsustainable job sustainable. Some of what people bring to work related stress counselling turns out to be a decision rather than a symptom, and part of the work is getting well enough to make it.

Executive burnout has its own texture. Decisions start taking longer than they used to. Work follows you into the evening without adding anything.
